Certificates of Insurance Request

A certificate of insurance (COI) is a document from an insurance provider confirming that Westminster School District is insured/protected.  It is also known as a certificate of liability insurance or evidence of insurance and is proof of coverage. It is required for when services are contracted/provided to or from WSD.     

Requesting a Certificate of Insurance                                                                                                                       

  1. WSD Vendor/Consultant Agreement outlines the required coverage.  
    • Refer to Section III B. Insurance Requirements
  2. Company requesting a COI from WSD.
    • Complete a Certificate of Insurance Request Form and email it with the company's agreement to APham@wsdk8.us.                                                                                                                                         

Please allow 5-7 business days for processing providing that there are no delays due to incomplete or missing information.
