Property or Liability Claim Submission

Claims submitted/presented are evaluated on their individual merits. Filing a claim does not mean it will be paid.

1. Westminster School District Claim Form Complete the form for personal claims and deliver it with wet signature (hard copy), photo identification, supporting documentation, and/or receipts for initial review/acceptance to the District Office - Business Services Division, attention Anne Pham, 714-894-7311 ext. 1012.
    
2. Report of Loss from Burglary, Theft or Vandalism Complete the form for losses incurred to WSD property. The supporting documentation; pictures, videos, and receipts will be sent with the completed form to the Business Services Division. Submit a Facilitron Work Order request, if needed to FMOT for any necessary repairs.
